Your first visit to our office includes:
- A review of your oral health history
- A comprehensive exam
- A customized treatment plan
- X-rays (if necessary)
- Cleaning (if necessary)
New Patient Forms
We will send a text message with a link for the necessary paperwork once your appointment is scheduled. If you don’t have access, arrive a few minutes before your appointment to complete the forms in person. Contact our office if you have any questions.
